We are seeking a dedicated and skilled Triage LPN to join our Pediatric team. The ideal candidate will have experience in a clinical setting. This role requires a strong understanding of medical terminology, patient care, and administrative support to ensure the smooth operation of our clinic. The Triage LPN will play a crucial role in providing high-quality care to patients while supporting physicians and other healthcare professionals.


Position Summary



The Clinical Triage Nurse serves as the first line of clinical communication for pediatric patients and families, providing expert assessment, guidance, and coordination of care. This role is responsible for evaluating symptoms, determining the urgency of medical needs, offering clinical advice, and ensuring timely access to appropriate care. The Clinical Triage Nurse plays a key role in supporting the patient’s experience and promoting safe, effective, family-centered care.



Essential Job Functions



  1. Conduct telephone and in-person assessments to determine the nature and urgency of pediatric patients’ medical concerns.
  2. Utilize pediatric-specific triage protocols and clinical judgment to identify appropriate care levels and prioritize appointments or referrals.
  3. Recognize urgent or emergent symptoms and direct families to emergency care when necessary.
  4. Gather and document accurate information from parents or guardians regarding symptoms, medical history, and current treatments.
  5. Collaborate closely with pediatricians, nurse practitioners, and other members of the care team to ensure appropriate follow-up and care coordination.
  6. Provide safe and timely clinical advice in accordance with established protocols and scope of practice.
  7. Assist in managing patient flow, ensuring urgent needs are addressed promptly.
  8. Offer education and reassurance to parents on common childhood illnesses, home care, and when to seek further medical attention.
  9. Reinforce provider instructions, treatment plans, and preventive care recommendations such as immunizations and wellness visits.
  10. Communicate with empathy and professionalism to support family understanding and trust.
  11. Accurately document all triage interactions, assessments, and advice provided in the electronic health record (EHR).
  12. Maintain patient confidentiality and comply with HIPAA and organizational privacy standards.
  13. Communicate clearly and effectively with clinical and administrative staff to support continuity of care.
  14. Schedule or adjust patient appointments based on clinical urgency and provider availability.
  15. Participate in quality improvement initiatives, staff meetings, and continuing education related to pediatric nursing and triage.
  16. Maintain up-to-date knowledge of pediatric clinical protocols, vaccines, and current healthcare guidelines.

Qualifications

Qualifications

  • Education: Graduate of an accredited Practical Nursing program.
  • Licensure: Current Licensed Practical Nurse (LPN) license in the state of Tennessee.
  • Experience: Minimum of two years of clinical experience in pediatrics, family practice, or emergency/urgent care preferred.
  • Skills:
    1. Strong clinical assessment and critical thinking abilities.
    2. Excellent communication and interpersonal skills, especially with parents and children.
    3. Proficiency with electronic health records (EHR) and telephone triage protocols.
    4. Ability to multitask and work effectively in a fast-paced clinical environment.
